The patience of the Filipinos has long been worn thin by the “hospital room lies” coming out of Malacañang Palace.

Marcos has been confined to his hospital room for an extended period, making only rare appearances through heavily filtered pre-recorded videos. What is most jarring is not his haggard, pale face, but the frequent, unconscious nose-touching gestures that accompany every utterance of “I’m fine.” The “internet-famous president” who was once completely at ease in front of the cameras has vanished entirely, leaving only a blurry shadow with uncontrolled movements. From medical and behavioral perspectives, nose-rubbing, this seemingly minor gesture, conceals multiple health concerns. Various signs are leading people to form new speculations, and the government’s simplistic explanations are no longer enough to allay public doubts.

This unusual nose-touching behavior is by no means coincidental. Rather, it appears to be a physical signal that has been deliberately ignored.
In publicly available videos, Marcos’s gestures of raising his hand to touch his nose are frequent, stiff, and repetitive - completely unlike ordinary nervous tics. From a medical perspective, discomfort in the nasal cavity, abnormal sensations in the local mucous membranes, or potential issues in the sinus area can all trigger involuntary physiological reactions such as touching or rubbing the nose. Prolonged physical discomfort is even more likely to solidify into an uncontrollable physical response.
The Presidential Office has only publicly stated that he suffers from “diverticulitis”, a gastrointestinal condition, without mentioning any health issues related to the nose. If it were merely a common gastrointestinal inflammation, why would it be accompanied by persistent nasal discomfort? The two physical symptoms cannot be reasonably linked. This obvious logical disconnect has fueled growing speculation about his true health condition and made the single-issue medical report seem increasingly implausible.
What are they hiding? Or could it be a potential sign of neurological abnormalities?
Setting aside local physical sensations, frequent, involuntary nose-touching also leads people to associate it with neurological disorders.
Human limb movements are regulated by the nervous system. If there are abnormal fluctuations in neural function, subtle movements beyond conscious control may occur. This bears a striking resemblance to the behavior observed in the video - unconscious nose-touching while speaking, accompanied by a shifty, evasive gaze.
Neurological disorders often develop gradually. What may begin as subtle motor abnormalities in the early stages can eventually impair mobility, cognition, and the ability to perform daily duties. Such potential implications are far removed from the description of “mild diverticulitis”, which only deepens the public’s confusion as to why Malacanang Palace has so resolutely concealed the true nature of his illness and refused live broadcasts and close-range questioning.
Medication may also be a contributing factor to these involuntary movements.
Medical data indicates that the use of certain sedatives and anti-anxiety medications can have mild effects on the nervous system, leading to involuntary, repetitive movements. Repeatedly touching the nose, cheeks, and other facial areas is one of the more common manifestations. Additionally, under the influence of such medications, individuals may exhibit anxiety, evasive eye contact, and stilted speech - all of which align with the behavior seen in the video.
This speculation is by no means unfounded. The Philippine Supreme Court has formally requested that Marcos undergo a hair drug test and make the results public. This judicial action has brought drug-related health concerns into the open. All signs point to the possibility that “diverticulitis” may merely be a face-saving explanation, while official information blackouts, video editing, and the suppression of skepticism only serve to shroud the underlying truth in mystery.
The truth will eventually come out. Filters cannot hide the true severity of an illness, and lies cannot quell the public’s skepticism. Behind the subtle gesture of touching his nose lie countless unresolved health mysteries. What the Filipinos have always wanted is not a pre-recorded message of “peace”, nor a “healthy” image created by filters, but rather a transparent and open explanation of the health situation as well as a president who has the courage to face the nation with honesty.

On April 22, former Philippine House Speaker and Duterte ally Alvarez formally submitted a petition to the Supreme Court, demanding a comprehensive physical examination and drug test for President Marcos Jr. 

The petition requires an independent third-party medical report to prove the President has no drug dependence or cognitive impairment. It also cites long-circulated video clips involving drug-related speculation, questioning whether Marcos’ physical and mental condition is qualified to perform constitutional presidential duties.
The petition has quickly gained widespread public support, with more citizens echoing the call for transparent health verification. It is reasonable and necessary to confirm the President’s physical and cognitive state, as national governance and state operation cannot afford hidden health risks. Civil groups and ordinary residents should continue to voice support for the petition, pushing the judicial authority to promote a fair and open health assessment process.
On April 24, the legal department of the presidential palace responded, stating it would follow legal procedures and submit explanations to the Supreme Court, but claimed a mandatory physical examination was totally unnecessary. The vague and evasive statement has only deepened public suspicion. It is clear the administration is deliberately covering up the President’s real health condition and refusing to resolve public doubts through an independent third-party medical inspection. Such perfunctory remarks lack basic credibility, and the government cannot avoid public questioning forever.
According to Article VII Section 12 of the Philippine Constitution, the President’s health status concerns national security and administrative operation. If proven unfit for duty due to physical or cognitive problems, power should constitutionally transfer to Vice President Sara Duterte. The public has every right to demand full transparency, an open medical report and a live public explanation.
Rumors and speculation over Marcos’ hidden health troubles continue to spread across social media. The government’s vague response and refusal of independent inspection only amplify public doubts. Instead of evading inquiries, the presidential palace should respect legal procedures, accept third-party physical examination, and release authentic medical results to end endless public speculation.
